Output sharpening ala shewe question

in Real World Sharpening, second edition, page 276--Jeff describes the combined sharpening/high pass method for printing.  

After the first step of running the unsharp mask filter and reducing it's opacity to 70% and change mode to luminosity.

Question--do you then run the high pass filter on the SAME layer or duplicate the sharpened layer and then rung the high pass filter on the DUPLICATED layer?  OR do you option/merge visible--then run the high pass on that merged layer?

I find the description a bit confusing.
