Photoshop CS6 Adjustment Layers Not Working

 I have several versions of PS installed on my PC. The version that I feel most comfortable with and use most is CS6. I must have recently inadvertently set something wrong, and “New Adjustment Layers” is not working properly. With an image loaded, if I hit LAYER>NEW ADJUSTMENT LAYER>, LEVELS>, a dialogue box pops up wanting me to name the layer, but the levels graph allowing me to make adjustments does not come, thus I cannot make adjustments. The same image loaded on a different version of Photoshop does work, so I screwed up CS6 somehow.
87 years old and sometimes I think I am really losing it.

Let's start with a reset of preferences to see if this fixes the issue.
Press and hold Alt+Control+Shift (Windows) or Option+Command+Shift (Mac OS) immediately after launching Photoshop. You will be prompted to delete the current settings. You can also reset preferences on quit if Photoshop is running by going into General Preferences>General>Reset on Quit.

I think he's suggesting that you just reset Prefs and see if that fixes the problem. If not, then you'll need to go to Plan B.

Incidentally, over at dpreview I gave you a solution that works. You replied, "It does not allow me to name the layer." But it does allow you to name the layer. See my reply.

Thanks all for your help. I think I tried everything suggested and I some how got it to work. Not sure how. Here is a screen shot.
