Pentax Lens on EOS problem

After some kind help on this forum I decided to of for the Pentax -A 645 55mm as an alternative to the 45mm tse.
I use the 17mm, 24mm and 90 tse's on Canon and wanted something around 45-55mm instead of the 45tse.

The Pentax came as an import from Japan and I also bought a Kippon shift adapter. So far it's not working out at all which is perhaps down to my inexperience. My workflow is as below but the images are out of focus and pretty awful. The attached test files include one from a Canon 28-70 and the warmer one from the Pentax. Any help at all would be very much welcome.

I'm working on a tripod.
I'm setting the aperture ring to f11. The live view appears almost black so I'm setting the ISO sky high so I can focus and then bringing it back down again to capture.
