Printing result messed up

Hey guys!  Quick question.  I'm using a Canon ipf8400 printer.  I have the right Lexjet Sunset Metallic paper type loaded on the printer.  Im printing from Photoshop using the ipf8400 Print Plugin.  I have the Sunset Photo Metallic paper selected on the media type.  See attached image of my workspace and settings.  I print the image and it looks horrible.  I've also attached the end result as well.  What am I doing wrong?

If you look at the hair the way it is printing typically means a black clog. What does your nozzle check look like?

I went ahead and selected the metallic paper output profile and result was the same result.

Whoah!  I think I found my problem    So it's a printhead issue for sure, right?
