FS Mamiya 645 AF 150 F/3.5, with lens hood and front and rear lens caps.

Excellent condition.  No marks on barrel and the glass is very clean.  I have tested this lens against my other Mamiya lens and another 150mm and found it to be very sharp.  Will package carefully for shipping.  Buyer must have verified PayPal address.
