I just tried printing from Lightroom 5 and I'm getting REALLY red skin tones on canvas. Reprint the same image using the same .icc profile, same settings in LR4 and it's perfect, same with printing from Photoshop CS5.
Anyone else notice this? Is there a fix?
Specs if you're interested:
MacPro 3,1 running OSX 10.8.4
Lightroom 5.0
Canon iPF8400
Custom profiles made with a ColorMunki

Sounds like double color management or the application of the profile twice. Check your color settings in both to make sure they are set identically.

Crap. Yes, that's exactly what seems to be happening.
There's color management built into the print driver and in Lightroom 4 and PhotoShop CS5 have it turned off when the application manages color, LR5 has it turned on when the application manages color and the option is grayed out and there doesn't seem to be a way to turn it off in the driver. I've attached a screen grab to show what's going on.
Nobody else printing with a Mac/Canon combo is seeing this issue?

Is there a dedicated 'Colour Management' window from the drop-down that's labelled 'Main'?

Yes, the "Color Matching" tab has options for ColorSync and Vendor Matching but if I'm using Lightroom to manage colours both options are greyed out.

They're greyed out, but the wrong "color" mode is applied instead of "No color correction", it's a common problem. Update AppColorMatchingInfo.xml file with Doyle's modified version:
