Has anyone used this iphone app succesfully?

I have, and I like the idea. But it is a lot more time consuming then the old pen and paper. That's because it it a lot more thorough with the information it gathers. The problem I have with it other then the extra time it takes is that I either have to interview the model to get all that extra info (date of birth, ethnicity, location, etc) or if I give it to them to fill out and they have to hand it back to me to take their embedded picture and finish the process. For that reason I went back to paper releases until the new ipad comes out with two cameras like the iphone.
