I want to determine the date that I added an image to a collection. Is there a way of doing this either for a regular collection or for a smart collection? Or can I determine the date I added a keyword to an image?

I think the only place you'd any find information like this, as standard, is in the develop module's history panel. It does show when the file was added to the catalogue.
I don't think the rest of that information you're asking about is recorded anywhere.

Perhaps creating a virtual copy and add that to the collection. Then the 'add time' may be 'creation time' for the image (would have to check that) or capture that time to some other field in the library panel

The creation time does appear as the first history entry for VCs.
