Title: License for web and social media?

Hello--

A website has requested permission to use my images on their site and in social media posts. I want to retain ownership of my originals and grant a perpetual, non-exclusive, limited-use license for electronic media only (not print). I also want attribution via credits or watermarks for my images.

Is there a standard license/release form someone can point me to? I've searched around and most of the agreements I find are for-hire type contracts.

I have mine generated through blink bid but you could look up the ASMP business practice book and it has a load of contract forms.
