Title: Authorization for one timwe use of Photo

A relative wants make a large version of one of my images for use in home.  The company making this large image requested an Authorization to copy and reproduce this image.  I have searched for a template that could be used, or other similar forms on the net but had no luck.

Can anyone direct me to where I could find such a form?

Never heard of such a form.

Two options:

1. Free style the authorization, e.g. "I, ______, herby authorize  ______ a one-time use of the photo ____...."

2. You order the print from your account and drop-ship to your relative's address

You should make it clear that the authorization is solely for the reproduction of the image for (name of recipient) and transfers no other rights or licenses.  The authorization automatically expires upon completion of the order and no test prints can be kept or transferred to anyone else.
