Does anyone know of a tool which identifies out-of-gamut colours in an image and substitutes the closest colours from a specified colour profile (i.e. relative rendering intent)? Ideally such a tool would quantify the Delta E for the mismatches (or at least for the maximum mismatch).

I suppose Colorthink by Chromix would be as close as you're going to get

ColorThink can plot images and their gamuts in relationship with other images or other profiles. You could build a color list and produce dE reports, the color lists could be two differing conversions for example (RelCol vs. Perceptual).
As to substituting the closest color, that' be the ICC profile itself.

As Andrew mentioned, the color management system using the ICC profile will map the out of gamut colors to the nearest match.
