I upgraded to LR 5, but when I click on the icon for LR 5, it opens as LR4 Catalog-2-2. The features that are supposed to be in LR 5 are not there. What am I/did I do wrong?

You can check the version of LR that is open on the Menu > Help > System Info...
A Catalog that has a suffix "-2" indicates it has been updated to the current version of Lightroom (regardless of the name!)
So in Lightroom4 your catalog was { LR4 Catalog-2.LRCAT } After the update in LR5 you will now have { LR4 Catalog-2-2.LRCAT } and the 'old' version remains as well!
You could change the name of your Catalog { LR4 Catalog-2-2.LRCAT } to avoid this confusion. Call it { My Master Catalog.LRCAT } or similar.

The Catalog is inconsequential in which application is started. Guessing this is Windows, either way Uninstall all LR instances and reinstall the latest one you want. LR6 is out BTW.
I never liked that LR would make a different Program Files folder for each version of LR. LR CC has stopped doing this and always installs to the same PF folder. Much better for avoiding these types of issues.
