I have not tried the IV yet. But I found this comparison by Nick Frank:

It seems that the new features are mostly a wider opening and a different coating to reduce reflections. But it seems as if metabones did not really solve the issue with image degradation with the TS-Es (

). According to Nicks tests with the TS-E 24 the flocked metabones III shows better performance than the IV with the new coating. So no good news. The IV might be the better option if you have none yet - because it has a wider opening. That will probably help if you want to flock it (with the metabones III I still get somes vignetting with full shift because of the flocking material gets into the light path). But if you already have a metabones III it seems not worth an upgrade.
