Back is in perfect condition and comes with all pictured.
Plus: Phase One 1 Year Uptime Assurance (Warranty + Back Loaner, like Value Added Warranty) until Sept 2014. This is tied to the back serial number and serviced by Phase One, not a specific dealer, so you have almost a year of warranty and loaner.

The p45+ is a perfect workhorse back that can work on a technical camera (not just Hasselblad V). I used this back with a Cambo RS and is a great match to the very good, but slightly less expensive non-retrofocus Schneider lens designs (like the Schneider 35mm). Once the back is sold I will have a spare Cambo mounting plate to let go as well. Of course if you happen to buy the back for use on a Cambo I'd happily include it for the free (the plates are ~$400 new).
