"It is impossible to convert the mount of a digital back for a different medium format body via adapter or conversion plates, because the sensor would then not sit within the film plane (since the sensor needs to sit the same distance from the lens that the film would). Such conversion may be possible by sending the back to Leaf to be physically reconfigured, depending on the specific model." - Wikipedia

From what I recall, the question has been posed a couple times and what happens when people have sent their backs in for a mount change, they get a completely different back.  I'm sure a couple of the folks will chime in, though.

Typically, Leaf digital backs undergo an actual mount change (due to the design of the Leaf chassis, which allows this). Phase One digital backs are swapped entirely for the same model in a different mount.
