from metadata in history step

I am using Lightroom 3.5 on a PC running Windows 7.  I have found several images in which there is a history step in the develop module stating 'from metadata.'  I cannot find what creates this step and what data is being saved in history.  I have tested several images where I have executed the 'save metadata' menu item both in the library module and the develop module.  In every execution this has NOT created an 'from metadata' entry in the develop history.  Just executing 'save metadata to file' does not create a history step.  Does anyone know what action or actions creates this entry and what data is being saved in the history step.

Isn't that rather "read metatadata from file"?

Yes, you are correct.  I was going from memory and had a memory lapse.  Anyhow it 'read metadata from file'.
