98% of my commercial work is shot tethered using the awful DPP software. live view is essential for me, I hardly ever look through the camera's viewfinder anymore. I have a 28" screen additionally to my computer just to get good live view.

I like the simplicity of the way lightroom tetheres, but is there any way to get live view while tethering with LR3? it doesn't seem to work once i got eos utiliy running. using the video out is not a real option, because it looks crap. I don't mind having one program for tethering besides LR running. I am on a Mac, Canon 1Ds3 and 5D2.

Not as far as I know.

I use the tethering when shooting directly in LR now, but if I need LV, or controls on the camera , I use EOS Utility.
I don't think you need DPP for Live View, I use it through EOS Utility and it records directly into a watched LR folder, then is imported into the LR catalogue.

You have to turn off the flash settings for the flash to work with the 5DMKII if using other than a Canon SpeedLight.

For tethered shooting with live view:

Sofortbild has live view for Mac with Nikon Cameras. Great for HDR photography. FREE.

DSLR Remote Pro has Live View also for Canon/Nikon with Windows and Nikon with Mac
and their NKRemote  works with Nikon on Windows. Paid applications.
