selection not matching path

I was using photoshop cs4 on someone elses laptop yesterday and found really strange behaviour that i have not encountered before. When I made a path, then converted the path to either a selection (or mask) the selection/mask did not match the shape of the path i made. Feather is set to zero and the pen mode is on "add to path area" as opposed to exclude/intersect. I am not in quick mask, there are no other paths, selections or alpha channels active. If i path, say, a square i might get a square but with a number of geometric shaped holes in the selection. I've restarted photoshop, ran applejack (cache cleaning, fsk, permissions repair etc) tried it on different files, disabled open gl, but the problem reamins. Please see the example images i mocked up that demonstrated approximately what is happenes.
