Double display

How can I get LR to start with a large image on one and thumbs on the other screen (in other words: how can I configure LR so that I don't have to press Shift-G every time I start LR)?

If you show the film strip along bottom then at top left of film strip there are two icons to represent each monitor. Click and hold to see possible options or click Mon 2 for toggling second screen on and off.
