I don't have any personal experience with them, but I hear good things about Blurb.com.  John Paul Caponigro even uses them for his Antarctica book.  That's a pretty decent endorsement IMHO.

I prefer Paperchase in LA.  Their website is www.paperchase.net.  They printed a book for me that looks great.  I had first tried Blurb but wasn't satisfied with their quality.  Jim
