No matter how much I mess with this, it never appears to be level.  Maybe it's the ceiling, or the front bumper is throwing it off?  Does it look off, or is it just me?  Any other comments would be appreciated as well.

Well, according to my copy of Lightroom, the train is level.  There are a lot of curves in this image, which may make things look off a little bit.  Also, with the curved ceiling, unless the apex is mid-frame exactly it will look higher on one side than the other and that may make it look off.

Other than that, I wouldn't crop in quite so tightly... YMMV

Two things you might want to play with are lens distort Filter menu and debarrelize (if that's a word) the image and then transform & distort (edit menu) You definitely didn't shoot it symmetrically straight on
