Fixing POSTERIZATION? Tips, Advice, Opions?

Lately I have been happy with the finish images after post-processing but still there are some moments when I'm happy with the image tone but when I check the Histogram I can see the sign of Posterization.

Anybody dealing with the same problem?

PS: When I'm working in 16 bit the effect is less pronounced although I have never been able to achieve to keep the look I got and fix the Posterization entirely. Are there any parameters I should be aware of?

What is important is whether you see posterization in your output images or not. Remember that histogram doesn't tell you anything about how the pixels of corresponding colors are distributed across the image, that is important for image perception (and possible posterization perception, too). If you see some spikes in the histogram, it doesn't necessary means that visible posterization will appear in, say, print.
