Epson 4880

I work with an Epson 4880, we have a new Eizo Flexscan EV3237 monitor which was calibrated with Spyder4Elite and running PS CS.
When I print the color is WAY off.
I've taken screenshots of all the settings, the images have  sRGB as the profile in PS.

I can't figure out what is set incorrectly.

Any ideas?
