While I was out of town a Precision One technician replaced head and capping station on Wednesday, and when I returned Friday the first thing I did was run a nozzle check and CYAN was completely clogged !!

The same thing happened a year ago when they replaced the same parts...

Could something else be causing this besides the head and capping station ?

Complete loss of a channel I feel is more a damper issue. You may have air in the line.

But call D1 and have them come back out. Work should be covered.

Someone posted in some other Epson -clogged-thread that the old ink had settled and solidified....and it was cyan-black. Something with the ink selector

As it seems you forgot to change the most important thing: the brand!  :P

Sorry, I could not resist to make the joke, but it somehow translates my thinking. It really sounds like air in the lines, but with epson... who knows?!
Any equipment with recurring problems is a lemon (IMHO). Head changing is a major intervention for an epson, if you changed the head twice and the problem persists it does not bode well.
