This is what I am working with, Phase One P25 Flex Adapter on 4x5 with Sinar Shutter.
My question is, what is the small adapter that connects the "wake up" cable and the P25 called? How much do they cost? Any options to modify another adapter to work the same?
Thanks

We've always just called them a "mutiport pigtail adapter"
You can order them through any Phase One dealer. Prices vary, as you might expect.

Kapture Group makes a cable that will work with your P25 / 4x5. It's called the One Shot Cable and it can wake up your DB and fire the shutter in one motion. There are two versions of this cable. One will come with a mini-phone tip for older H series backs and the second is for newer P series backs that will have a multiport connection. It is $395.00 and we have them in stock.
